Help Improve Lives Through Whole-Person Care
At Klamath Basin Behavioral Health, we believe mental wellness and physical health go hand in hand. We are seeking a compassionate, clinically skilled Nurse (LPN or RN) who excels at building relationships, coordinating care, and helping individuals successfully manage both mental health and medical needs.

This is not a task-focused nursing role. Success in this position comes from helping clients remain engaged in treatment, identifying health concerns early, coordinating care across providers, and ensuring clients receive the support they need to improve their overall well-being.

What Success Looks Like
In this role, you will:

  • Serve as a trusted clinical resource for clients receiving psychiatric services.
  • Conduct effective telephone and in-person triage while making sound clinical decisions within your scope of practice.
  • Identify medical concerns, medication side effects, and health risks before they become larger problems.
  • Coordinate care with primary care providers, specialists, hospitals, community partners, and caregivers to improve client outcomes.
  • Help clients successfully manage chronic conditions such as diabetes, COPD, and Hepatitis through education, monitoring, and skills training.
  • Administer medications and injections accurately and safely.
  • Support psychiatric providers by ensuring critical medical information is communicated and acted upon appropriately.
  • Maintain accurate clinical documentation within required timelines.
  • Contribute to a collaborative, recovery-oriented treatment environment.

How Performance Will Be Measured
Successful candidates consistently demonstrate:

Appropriate triage and assessment decisions.

  • Timely follow-up on abnormal laboratory results and medical concerns.
  • Effective identification and communication of medication-related issues.

Clinical Judgment

  • Appropriate triage and assessment decisions.
  • Timely follow-up on abnormal laboratory results and medical concerns.
  • Effective identification and communication of medication-related issues.

Client Engagement

  • Strong therapeutic relationships with clients.
  • High levels of client participation in treatment and wellness activities.
  • Compassionate and professional interactions with diverse populations.

Care Coordination

  • Effective collaboration with internal and external healthcare providers.
  • Successful connection of clients to medical resources and community supports.
  • Proactive communication that improves continuity of care.

Documentation & Compliance

  • Timely completion of clinical documentation.
  • Accurate and thorough medical records.
  • Compliance with agency, state, and federal standards.

Team Contribution

  • Active participation in team meetings and trainings.
  • Willingness to share nursing expertise with interdisciplinary staff.
  • Contributions that improve workflow, client care, and service quality.

The Ideal Candidate
You are likely to thrive in this role if you:

  • Enjoy working with individuals experiencing mental health challenges.
  • Balance compassion with strong clinical decision-making.
  • Are highly organized and able to manage multiple priorities.
  • Communicate effectively with clients, families, providers, and community partners.
  • Remain calm and professional during crises and challenging situations.
  • Value collaboration and understand the importance of integrated care.
  • Take ownership of your work and follow through on commitments.

Qualifications
Required

  • Active Oregon LPN license.
  • Minimum of two years of professional nursing experience.
  • Valid Oregon driver's license and acceptable driving record.
  • Ability to travel throughout the community and client service area.

Preferred

  • Experience in behavioral health, psychiatric nursing, community mental health, or integrated healthcare settings.
  • Experience working with adult populations.
  • Knowledge of psychotropic medications and medication monitoring.
  • Experience coordinating care across multiple healthcare systems.

As a member of our team, your work directly supports individuals on their recovery journey and helps ensure they receive the medical and behavioral health services needed to live healthier, more independent lives.

If you are a nurse who enjoys building relationships, coordinating care, educating clients, and making a meaningful impact in your community, we'd love to hear from you.
