Job Summary:

We are seeking a compassionate and skilled Bilingual Nurse Practitioner (NP) fluent in Mandarin and/or Cantonese to provide high-quality, patient-centered care through in-home visits in the New York Metro Area. The Rendr@Home Nurse Practitioner will deliver comprehensive primary care, chronic disease management, and preventive health services to a diverse patient population, with a focus on serving Mandarin and/or Cantonese speaking communities. This role requires excellent clinical expertise, cultural competence, and the ability to work independently in a home-based setting. 

Key Responsibilities:

  • Conduct in-home assessments, including physical exams, medical histories, and diagnostic evaluations for patients, with a focus on geriatric and chronic care populations.
  • Develop and implement individualized care plans in collaboration with patients, families, and interdisciplinary healthcare teams.
  • Manage chronic conditions (e.g., diabetes, hypertension, heart disease) and provide preventive care, health education, and counseling.
  • Order, interpret, and follow up on diagnostic tests, labs, and imaging as needed.
  • Prescribe medications and treatments in accordance with New York State regulations and scope of practice.
  • Administer immunizations and conduct appropriate health screenings and wellness checks
  • Address urgent medical needs in the home and provide timely follow-up care for patients recently discharged from the hospital, emergency department or skilled nursing facility to prevent readmission.
  • Communicate effectively with patients and families in Mandarin and/or Cantonese to ensure clear understanding of medical conditions, treatment plans, and follow-up care.
  • Coordinate care with primary care providers, specialists, and community resources to ensure continuity of care.
  • Document patient encounters accurately and timely in electronic health record (EHR) systems.
  • Provide culturally sensitive care, addressing the unique needs of Mandarin- and/or Cantonese-speaking patients.
  • Participate in quality improvement initiatives and maintain compliance with organizational policies, state regulations, and CMS guidelines.
  • Travel to patients’ homes within the New York Metro Area, maintaining a flexible schedule to accommodate patient needs.
  • Participate in network activities such as provider meetings, grand rounds and training sessions.

Qualifications:

  • Active NYS PA/APN license required.
  • DEA Required.
  • BLS/ACLS certification required.
  • Board Certified.
  • Minimum of 1-2 years of experience as an Nurse Practitioner or Physician Assistant, preferably in primary care, geriatrics, or home health. Experience with in-home visits or community-based care is a plus.
  • Bilingual English and Chinese (Cantonese) required.
